**Offline**
**Step 1:** The interested applicant should take print of the [prescribed format](https://sw.cg.gov.in/sites/default/files/divyangscholarshipscheme.pdf) of the application form available on the official website of the Social Welfare Department, Government of Chhattisgarh.
**Step 2:** In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).
**Step 3:** Submit the duly filled and signed application form along with the documents, within the prescribed period (if any), to the **District Office of Social Welfare / Janpad Panchayat Office** through the Head of the institution.
