**Step 01:** The applicant should take print of the [prescribed format](https://socialwelfare.tripura.gov.in/sites/default/files/Notification%20of%20Scheme%20for%20providing%20pension%20to%20the%20Cancer%20Patients_0.pdf) of the application form.
**Step 02:** In the application form, fill in all the mandatory fields, paste the passport-sized photograph (signed across, if required), and attach copies of all the mandatory documents (self-attest, if required).
**Step 03:** Application should be submitted to the Office of the Child Development Project Officer (CDPO) of the concerned area.
**Step 04:** Request a receipt or acknowledgement from the concerned authority to whom the application has been submitted.
Ensure that the receipt contains essential details such as the date and time of submission, and a unique identification number (if applicable).
